Where It All Began — The EMR Era

RiskAssistMD began in 1999—when digital charts replaced paper, and everyone learned to love… clicking. EMRs captured data, but they couldn’t see patterns or prevent risk. Follow-up lived outside the system, on sticky notes, and patients fell through the cracks. That problem sparked everything that came next.

The Rise of Lawsuits & Weak Clinical Processes

By the early 2010s, malpractice claims were rising—not from lack of care, but from missed follow-ups and silent failures.

Manual work and fragmented processes led to gaps, delays—one missed step could cost six figures. That’s when RiskAssist became a protective layer—catching gaps before they became patient harm or legal risk.

EMRs were built for documentation—not for managing care-pathways or surfacing risk. RiskAssist introduced automated workflows to identify overdue follow-ups and unaddressed gaps. Providing clinics with clear visibility into risk—and the ability to systematically close critical gaps.

By 2020, RiskAssist was identifying more cases than clinics could manage. CMOs told us, “We can’t handle this volume.” So we shifted focus—from detecting risk to enabling action. RiskAssist built a patient navigation platform—automating follow-up by disease pathway to scale beyond staff capacity. The goal shifted from finding to resolving problems —at scale.
